Positive Fluid Balance Worsens Outcome in Septic Shock Fluid Resuscitation in Septic Shock: A Positive Fluid Balance and Elevated Central Venous Pressure Are Associated With Increased Mortality
نویسنده
چکیده
Background: The mainstays of early treatment in septic shock (antibiotics, infection source control, lung protective ventilation, and aggressive volume resuscitation) remain effective. However, although early goaldirected volume resuscitation and a target central venous pressure (CVP) of 8 to 15 mm Hg has become accepted practice, exactly how much fluids to administer or when to stop has yet to be defined.

Fluid resuscitation in septic shock: a positive fluid balance and elevated central venous pressure are associated with increased mortality.
OBJECTIVE To determine whether central venous pressure and fluid balance after resuscitation for septic shock are associated with mortality. DESIGN We conducted a retrospective review of the use of intravenous fluids during the first 4 days of care. SETTING Multicenter randomized controlled trial. PATIENTS The Vasopressin in Septic Shock Trial (VASST) study enrolled 778 patients who had s...
متن کاملFluid balance and cardiac function in septic shock as predictors of hospital mortality
INTRODUCTION Septic shock is a major cause of morbidity and mortality throughout the world. Unfortunately, the optimal fluid management of septic shock is unknown and currently is empirical. METHODS A retrospective analysis was performed at Barnes-Jewish Hospital (St. Louis, Missouri). Consecutive patients (n = 325) hospitalized with septic shock who had echocardiographic examinations perform...
متن کاملFluid therapy for septic shock resuscitation: which fluid should be used?
Early resuscitation of septic shock patients reduces the sepsis-related morbidity and mortality. The main goals of septic shock resuscitation include volemic expansion, maintenance of adequate tissue perfusion and oxygen delivery, guided by central venous pressure, mean arterial pressure, mixed or central venous oxygen saturation and arterial lactate levels. An aggressive fluid resuscitation, p...
متن کاملFluid administration in critically ill patients with acute kidney injury.
Physiologic end points for fluid resuscitation in septic shock patients with acute kidney injury (AKI) have been undertaken in randomized studies using the Early Goal-Directed Therapy (EGDT) approach. These studies have demonstrated a beneficial effect on in-hospital mortality with EGDT. The Saline versus Albumin Fluid Evaluation (SAFE) randomized study in critically ill patients demonstrated n...
متن کاملPassive leg raising test with minimally invasive monitoring: the way forward for guiding septic shock resuscitation?
BACKGROUND Swift and adequate fluid loading is a cornerstone of septic shock therapy. Yet, careful assessment of volume responsiveness and volume amount during the resuscitation process is a prerequisite. Both overzealous initial fluid administration and late fluid overload are harmful and may be associated with increased mortality. MAIN BODY Static (i.e., central venous or pulmonary artery o...
